We are so excited to host in-person activities! Due to the serious nature of the current COVID-19 pandemic, we must adhere to certain rules and behaviors in order to keep our in-person activities happening. If you, or any member of your household, are feeling sick, please stay home and join us next time!
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend physical distancing and have, in many locations, prohibited the congregation of groups and people. Wasatch Home Educators Network (W.H.E.N.) has put in place preventative measures to reduce the spread of COVID-19; however, W.H.E.N. cannot guarantee that you, your children, or any other person, will not become infected with COVID-19. Further, attending W.H.E.N. sponsored activities could increase your risk and your child or children’s risk of contracting COVID-19.
By purchasing tickets to this event, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child or children and I may be exposed to or infected by COVID-19 by attending W.H.E.N. activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at W.H.E.N. activities may result from the actions, omissions, or negligence of myself and others, including but not limited to W.H.E.N. volunteers, participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or children or myself, including, but not limited to, personal injury, disability, death, illness, damage, loss, claim, liability, or expense or any kind, that I or my child or children may experience or incur in connection with my child or children’s attendance at W.H.E.N. activities or events. On my behalf and on behalf of my child or children, I hereby release, covenant not to sue, discharge, and hold harmless and indemnify W.H.E.N., its directors, representatives, and volunteers, of and from any and all claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the W.H.E.N., its directors, representatives, and volunteers, whether a COVID-19 infection occurs before, during, or after participation in any W.H.E.N. program.
HOLD HARMLESS AGREEMENT: As a registered participant, or legal guardian of a registered participant, in any activity offered or sponsored by W.H.E.N., I recognize and acknowledge that there exist certain inherent risks of physical injury and I agree to assume the full risk of any injuries, including death, damages or loss which I, or the listed dependents, may sustain as a result of, or in any way connected with participating in any and all registered activities. I do hereby fully release and discharge W.H.E.N., its directors, representatives, and volunteers from any and all claims from injury, including death, damages or loss which I, or the listed dependents, may have or incur as a registered participant in any activity offered or sponsored by W.H.E.N. I further agree to indemnify and hold harmless W.H.E.N., its directors, representatives, and volunteers from and against any, and all, liability which may be suffered by myself or my listed dependent as a result of, or in any way connected with participation as a registrant in any and all activities offered or sponsored by W.H.E.N.
